Constellation Health Services in Cincinnati, OH seeks a compassionate Hospice RN Case Manager to join our team. You will provide skilled nursing care in patients' homes, assisted living facilities, and skilled nursing facilities, coordinating care with families and interdisciplinary teams.

Qualifications

• Graduate of an accredited RN program.
• Registered Nurse license in the state of employment.
• At least 1–2 years of RN experience is required.
• Previous hospice care experience strongly desired.
• Basic knowledge of iOS operating system.

Responsibilities

• Provides skilled nursing care including administration of medications and treatments as prescribed in the physician's plan of care for patients in their home setting.
• Performs a comprehensive assessment and coordinates the plan of care with all disciplines involved, including family and caregivers.
• Observes, evaluates and reports on the patient's status to the nursing supervisor and attending physician when changes occur.
• Participates in weekly Interdisciplinary Team Meetings and provides updates on patient status.
• Maintains and processes all documentation in compliance with agency standards using the computer-based documentation system.
• Provides input to Home Health Aide Supervisor for hospice aide evaluations as needed.
• Weekend and holidays coverage as needed.
