Job summary

Constellation Health Services in Houston seeks a Nurse Practitioner to provide hospice medical care in patients' homes under the supervision of a collaborating physician and in compliance with the state Nurse Practice Act.

You will perform comprehensive assessments, manage terminal illnesses, and develop individualized plans of care, coordinating with the hospice interdisciplinary team to ensure compassionate, high-quality end-of-life care for patients and families.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current DEA license and state nursing licensure, plus CPR certification.
• Hospice/home health experience preferred; accreditation from a certifying body for advanced practice nursing.
• Certification in Hospice and Palliative Nursing (CHPN) or similar preferred.

Responsibilities

• Provide medical care and manage terminal illness under supervision of a collaborating physician.
• Perform history and physical exams, interpret tests, and document findings.
• Develop and modify individualized plans of care, prescribe medications, and coordinate hospice services.
• Complete hospice face-to-face visits to assess eligibility for benefit periods.
• Collaborate with hospice medical director on patient eligibility for certification/recertification.
• Coordinate patient care with the interdisciplinary hospice team.
• Educate patients and families on care plans, symptom management, and end-of-life issues.
• Ensure accurate clinical notes and maintain medical records.
• Maintain compliance with HIPAA and Medicare/Medicaid rules for hospice services.
• Drive program growth by establishing relationships with staff and community referrals.
