Job summary

Gentiva is seeking a Nurse Practitioner to provide advanced, patient-centered care across Hospice and Palliative Care. You will manage patient visits, conduct Face-to-Face encounters for CMS certification, and lead symptom management with autonomy within physician-led teams.

You will collaborate with IDG/IDT and PCPs to develop care plans, order tests, and ensure compliance with Medicare/Medicaid standards while delivering compassionate end-of-life care in a fast-paced setting.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete all visit types for hospice patients including F2F visits within CMS timeframes.
• Collaborate with Medical Directors and IDG/IDT to deliver patient-centered care.
• Coordinate Plan of Care with PCP/attending providers and specialists.
• Perform in-depth patient assessments across physical, emotional, functional, and psychosocial domains.
• Prescribe medications and treatments within state regulations and CPPA parameters.
• Order and interpret labs and tests within scope of practice.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across home, SNF, and assisted living settings.
