Job summary

Gentiva seeks a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary home health team. You will provide advanced palliative care to patients in their homes, supporting quality of life and autonomy.

You will work with the Palliative Medical Director, conduct comprehensive assessments, create individualized care plans, and participate in call rotations as needed. The role requires an MSN, NP licensure, and two years of NP experience, with Part B billing experience

Qualifications

• MSN degree from an accredited school.
• NP-licensure and state certification where required.
• Experience with Part B billing is preferred.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Deliver comprehensive home-based palliative care to patients.
• Perform in-depth patient assessments including physical and psychosocial aspects.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on progress and needs.
• Prescribe medications (including controlled substances) as permitted by state law.
• Maintain timely, accurate clinical documentation in line with standards.
• Communicate with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals to additional care services when appropriate.
