Gentiva Hospice in the United States is seeking a Nurse Practitioner to provide dual hospice and palliative care. The role blends autonomy and collaboration, performing hospice Face-to-Face visits and managing patient plans of care with the IDG/IDT.

The ideal candidate holds an MSN or higher, NP certification, and has 2+ years of NP experience with hospice/palliative exposure; Part B billing familiarity is preferred. This safety-sensitive, flexible position offers comprehensive benefits.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited nurse practitioner program.
• Minimum of two years NP experience preferred.
• Hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Perform hospice F2F visits for eligibility and recertification.
• Collaborate with IDG/IDT to deliver patient-centered care.
• Coordinate care with PCPs and specialists as needed.
• Conduct in-depth patient assessments across physical, emotional, and psychosocial domains.
• Document findings and maintain compliant medical records.
