Job summary

Gentiva seeks a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team, delivering advanced home-based palliative care and guiding patients and families through the treatment journey.

You will work autonomously with the Palliative Medical Director, perform thorough assessments, and develop individualized care plans while managing complex medical needs and medications per state law. A strong emphasis on documentation and collaboration with clinicians is required.

Qualifications

• Master of Science in Nursing from an accredited program.
• Minimum two years of Nurse Practitioner experience.
• Family or Adult Medicine NP specialty preferred.
• Active NP license and RN license per state law.
• Experience with Part B billing.

Responsibilities

• Collaborate with the Palliative Medical Director to deliver patient-centered care.
• Perform comprehensive patient assessments (physical, emotional, psychosocial).
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans as patient needs evolve.
• Prescribe medications and order diagnostic tests as allowed by state law.
• Maintain accurate clinical documentation per professional standards.
• Communicate effectively with physicians, caregivers, and other healthcare professionals.
• Participate in evening/weekend on-call rotation.
• Facilitate referrals for additional care services.
