Job summary

Gentiva Hospice is seeking a Nurse Practitioner to deliver comprehensive, patient-centered hospice and palliative care. You will perform management visits, hospice Face-to-Face (F2F) encounters for eligibility certification and recertification, and collaborate with physicians and the IDG to ensure high-quality, compliant care.

The role blends autonomous practice with collaboration, allowing you to guide patients and families through complex medical decisions, symptom management, and end-of-life

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Perform hospice Face-to-Face (F2F) visits and manage CMS-required visits within regulatory timeframes.
• Collaborate with Hospice and Palliative Medical Directors and IDG/IDT to deliver comprehensive, patient-centered care.
• Coordinate patient Plan of Care with PCP/attending providers and specialists.
• Conduct in-depth patient assessments across physical, emotional, functional, and psychosocial domains.
• Support individualized care plans with patients, families, and caregivers.
• Prescribe medications and treatments within state law and practice agreements.
• Coordinate care across home, SNFs, and assisted living; facilitate care transitions.
• Document findings accurately and attest encounters in the medical record.
