Job summary

Gentiva Hospice is seeking a Nurse Practitioner to serve in a dual hospice and palliative care role. You will provide comprehensive, patient-centered care across the continuum of serious illness, collaborating with IDG/IDT and specialists to coordinate plans of care.

This flexibility supports patient needs and census fluctuations across settings. You will perform full scope NP duties, including F2F visits, symptom management, and end-of-life discussions, with emphasis on quality-of-life

Qualifications

• MSN degree from an accredited program and completion of an accredited NP program.
• Minimum of two years NP experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.
• Current RN license and NP license/certification in state of practice.
• National NP certification (ANCC or AANP).
• Collaborative Practice Agreement or Independent Practice Authority as required by state.
• Controlled Substance Agreement and DEA registration as applicable.
• Current CPR certification.

Responsibilities

• Complete all visit types for patients on services, including hospice F2F visits within regulatory timeframes.
• Collaborate with Hospice and Palliative Medical Directors and IDG/IDT to deliver patient‑centered care.
• Coordinate patient Plan of Care with PCP/Attending Providers and specialists.
• Perform in-depth assessments across physical, emotional, functional, and psychosocial domains.
• Support individualized care plans with patients, families, and care teams.
• Prescribe medications within scope and manage treatment plans.
• Order and interpret laboratory tests within state scope and CPP agreements.
• Facilitate goals-of-care discussions and advance care planning.
• Coordinate care across home, SNFs, and assisted living; initiate referrals.
• Document findings accurately and comply with regulatory standards.
• Communicate with IDG/IDT; provide clinical recommendations.
• Maintain knowledge in hospice and palliative care and ensure compliance.
