Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced, in-home palliative care. You will work autonomously with the Palliative Medical Director, guiding patients and families through each stage of treatment and collaborating with an interdisciplinary team.

The role requires MSN/NP certification, 2+ years of NP experience, and knowledge of palliative care, Medicare/Medicaid regulations, and Part B billing. Travel may be involved.

Qualifications

• Master of Science in Nursing or equivalent per state law.
• Minimum two years of Nurse Practitioner experience.
• Specialty in Family or Adult Medicine preferred.
• Experience with Part B billing.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Collaborate with Palliative Medical Director and care team to deliver comprehensive, patient-centered palliative care.
• Perform in-depth patient assessments (physical, emotional, psychosocial).
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on patient progress.
• Prescribe medications (including controlled substances) and order diagnostic tests per state law.
• Maintain timely, accurate clinical documentation.
• Communicate effectively with physicians, caregivers, and other health professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
