Job summary

Gentiva Hospice is seeking a Nurse Practitioner to deliver dual hospice and palliative care, serving patients across the continuum of serious illness. You will perform F2F visits for eligibility and recertification, collaborate with IDG/IDT, and manage complex symptom relief and end‑of‑life care.

This role blends autonomy with teamwork, requiring strong clinical assessment, empathy, and familiarity with Medicare/Medicaid requirements. A valid NP license and prescribing authority are essential.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Perform Medicare-required hospice Face-to-Face (F2F) visits and other CMS visit types.
• Collaborate with IDG/IDT to deliver patient-centered care.
• Coordinate care plans with PCPs and specialists.
• Conduct comprehensive patient assessments across physical, emotional, functional, and psychosocial domains.
• Prescribe medications and treatments within state scope and practice agreements.
