Job summary

Gentiva is seeking a compassionate Palliative Care Nurse Practitioner to provide advanced, home-based palliative care in collaboration with the interdisciplinary team, guiding patients and families toward the highest quality of life. You will work autonomously in partnership with the Palliative Medical Director and participate in call rotations as needed.

Requirements include a Master of Science in Nursing, NP certification, at least two years of NP experience, and familiarity with Part B

Qualifications

• MSN required and NP certification/licensure per state law.
• Minimum two years’ NP experience.
• Family or Adult Medicine specialty preferred.
• Experience with Part B billing.
• Ability to work autonomously in an advanced practice role.

Responsibilities

• Collaborate with care team to deliver comprehensive palliative care.
• Perform in-depth patient assessments.
• Develop and implement individualized care plans.
• Adjust treatment plans based on patient progress.
• Prescribe medications and order tests as allowed by state law.
• Maintain accurate clinical documentation.
• Communicate with physicians, caregivers, and team.
• Participate in evening/weekend call rotation.
• Facilitate referrals for additional services.
