Job summary

VIA Health Partners is seeking a Registered Nurse to provide nursing services in homecare or facility settings in Lincolnton, NC. You will coordinate care with an interdisciplinary team and educate patients and families on palliative care while ensuring quality, compassionate support.

Requires an active RN/LPN license, a nursing degree (2-year minimum), CPR, and 2 years of direct patient care experience. Travel to diverse care settings is expected as part of the role.

Qualifications

• A Diploma or associate degree in nursing is required; bachelor’s degree preferred.
• Current, active, and unrestricted RN/LPN license.
• CPR certification required; CHPN preferred.
• Minimum 2 years related experience in direct patient care.

Responsibilities

• Deliver nursing care within NC Board of Nursing scope of practice.
• Coordinate plan of care with interdisciplinary team to meet patient needs.
• Educate patient and family about palliative care and care plan.
• Maintain accurate clinical documentation per VIA Health Partners guidelines.
