Job summary

VIA Health Partners in Stanly County, Albemarle, NC, is seeking a dedicated Registered Nurse to coordinate patient and caregiver care within our hospice and palliative care program. The role requires an active RN license, a minimum of two years in direct patient care, and a diploma or associate degree in nursing.

The nurse will perform assessments, visit patients, provide interventions, and collaborate with an interdisciplinary team to meet care goals.

Qualifications

• Diploma or Associate's Degree in nursing is required; Bachelor’s Degree preferred.
• Current CPR certification required; CHPN preferred.
• Minimum 2 years of related experience in direct patient care.
• Experience with hospice/palliative care is a plus.
• Must be proficient with computer systems used for patient records.

Responsibilities

• Assumes responsibility for coordinating patient and caregiver care.
• Conducts nursing assessments to support care planning.
• Makes visits to provide interventions and revises plans as needed.
• Engages in emergency triage by telephone and home visits.
• Maintains professional relations with external agencies and healthcare providers.
• Keeps clinical documentation current per guidelines.
• Keeps medication profiles updated and educates patients and caregivers.
