Job summary

VIA Health Partners in Charlotte, NC seeks a dedicated Registered Nurse to deliver comprehensive hospice and palliative care. You will coordinate patient/caregiver care, perform assessments, and develop plans of care to ensure seamless service delivery.

Responsibilities include educating families, collaborating with an interdisciplinary team, and maintaining precise documentation. A Diploma/Associate in Nursing with an active RN license is required; Bachelor's degree is preferred.

Qualifications

• Nursing diploma or associate degree required; Bachelor's degree preferred.
• Current RN license in good standing; multistate licensure (NLC) preferred.
• CPR certification required; CHPN preferred.
• Minimum 1 year inpatient unit experience; 2 years in other nursing positions.
• Hospice/palliative care experience preferred; computer proficiency required.

Responsibilities

• Coordinate and deliver patient/caregiver care; conduct nursing assessments and update care plans.
• Educate patients and families; promote independence and adherence to care goals.
• Communicate with IDG and health professionals to ensure continuity of care.
• Maintain accurate, timely clinical documentation per VIA guidelines.
• Demonstrate professional development; maintain licensure, CPR and CEU requirements.
• May perform additional IPU duties and keep patient areas clean and stocked.
