Job summary

VIA Health Partners in Charlotte, NC seeks a dedicated Registered Nurse to join our night shift team for hospice and palliative care. You will coordinate patient and caregiver care, perform nursing assessments, visits, and revise plans of care to meet goals, while collaborating with an interdisciplinary team.

Applicants should have a nursing diploma or associate degree, an active RN license, CPR, and at least 2 years of related experience in life-limiting illnesses.

Qualifications

• Diploma or AA in Nursing required; BSN preferred.
• Active RN license in North Carolina.
• CPR certification required; CHPN preferred.

Responsibilities

• Coordinate patient/caregiver care and conduct nursing assessments.
• Visit patients to provide interventions and revise plans of care.
• Develop and evaluate plans of care addressing education, therapeutic and palliative nursing actions.
• Emergency triage by telephone and home visits.
• Maintain infection control and comprehensive clinical documentation.
