Gentiva Hospice seeks an experienced Nurse Practitioner to deliver comprehensive hospice and palliative care to patients in Georgia. You will conduct management visits, perform Medicare-required Face-to-Face encounters for eligibility, and partner with physicians and interdisciplinary teams to ensure compliant, patient-centered care.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred; hospice/palliative care experience preferred.

Responsabilités

• Perform hospice/palliative care visits, including F2F certifications and recertifications.
• Collaborate with IDG/IDT to develop patient plans of care.
• Order and interpret tests within state scope; manage treatments and medications.
• Document findings accurately and ensure compliance with regulations.
• Coordinate care across home, SNF, and assisted living settings.
