Job summary

Ennoble Care seeks an experienced Per Diem Nurse Practitioner to provide face-to-face assessments for hospice patients in the Atlanta, GA region. You will apply medical techniques and principles under the supervision of a physician, caring for a diverse group of patients with chronic diseases.

The role requires reviewing eligibility, certifications, and prior notes, reconciling medications, and communicating recommendations to the hospice physician/IDT as needed.

Responsibilities

• Provide face-to-face assessments for hospice patients in the Atlanta, GA region.
• Apply medical techniques and principles under supervision of a physician.
• Review patient hospice eligibility, terminal diagnosis, and prior certifications.
• Reconcile medications and review comfort medications and effectiveness.
• Educate patient and family on disease progression and hospice goals of care.
