Job summary

Gentiva Hospice seeks a Nurse Practitioner to deliver comprehensive hospice and palliative care across Marengo, Sumter, Greene & Hale Counties. You will perform F2F visits for eligibility and recertification, collaborate with IDG/IDT, and support families through serious illness and end-of-life decisions.

This PRN role blends autonomy with team collaboration, requiring an NP license, MSN, CMS knowledge, and a Collaborative Agreement with physicians; on-call coverage is required as needed.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• RN and NP licenses in state of practice.
• National NP certification (ANCC or AANP).
• Collaborative Practice Agreement or Independent Practice Authority.

Responsibilities

• Complete all hospice visit types within CMS timeframes.
• Collaborate with Hospice/Palliative Directors and IDG/IDT.
• Coordinate patient Plan of Care with PCPs and specialists.
• Perform in-depth patient assessments (physical, emotional, functional).
• Prescribe medications and treatments as allowed by state law.
• Order/interpret tests within scope of practice.
• Document findings and attest F2F encounters accurately.
• Participate in on-call rotation as needed.
