Job summary

Gentiva is seeking a Nurse Practitioner to provide comprehensive, patient-centered hospice and/or palliative care. You will perform Medicare-required Face-to-Face visits, manage assessments, and collaborate with physicians and IDG/IDT to ensure compliant, compassionate care.

The role requires NP licensure and MSN or higher, with experience in hospice/palliative settings; autonomy balanced with teamwork, and strong documentation. Travel and on-call may be involved, with benefits offered.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice or palliative care preferred.
• Current RN and NP licenses; NP certification.

Responsibilities

• Perform hospice F2F visits for eligibility and recertification.
• Collaborate with hospice/PM and IDT to deliver patient-centered care.
• Coordinate plans of care with PCP and specialists.
• Prescribe medications within scope and regulatory guidelines.
