Job summary

Gentiva Hospice is seeking a Hospice Nurse Practitioner to perform face-to-face encounters essential for eligibility certification and recertification, with a flexible, less-than-full-time schedule. You will gather comprehensive clinical information, assess patients, and collaborate with hospice physicians and the IDG to ensure compliant, high-quality care.

Qualifications

• Master’s degree in Nursing from an accredited program.
• Current RN license in the state of practice.
• Current APRN license or certification.
• National NP certification (ANCC or AANP).
• Collaborative practice agreement in place as required.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Knowledge of hospice care principles and regulatory guidelines.
• Familiarity with LCD criteria and hospice documentation requirements.

Responsibilities

• Perform hospice F2F physical assessments within CMS timeframes.
• Document findings and attest F2F visits in medical records.
• Apply LCD criteria to hospice documentation.
• Communicate findings with certifying physicians.
• Collaborate with physicians and IDG, patients, families.
• Provide clinical recommendations based on assessments.
• Conduct visits in patients’ homes or other settings.
• Notify agency if unable to complete visits.
• May provide coverage for multiple provider numbers with approval.
