Gentiva is seeking a Nurse Practitioner Palliative CPOM to lead comprehensive palliative care for its patients, delivering individualized treatment plans and ethical, clinical judgment under the Medical Director's direction as required by state law.

The role requires MSN or equivalent, at least two years of NP experience, and preference for Family or Adult Medicine specialty. Travel of 10-20% for training is expected, plus EMR proficiency and strong collaboration across disciplines.

Qualifications

• MSN or equivalent and NP licensure in good standing.
• Minimum 2 years NP clinical experience; palliative focus preferred.
• Ability to work autonomously in an advanced practice role.
• Experience with value-based care models and Part B billing preferred.

Responsabilités

• Lead palliative care for patients with individualized plans and follow-ups.
• Provide diagnosis, treatment and consultations under supervision as needed.
• Collaborate with the Palliative Medical Director and interdisciplinary team.
