Job summary

Heartland Hospice is seeking a Nurse Practitioner to provide comprehensive hospice and palliative care, delivering advanced clinical care, patient management visits, and F2F eligibility encounters. You will collaborate with IDG/IDT teams and physicians to ensure high-quality, compliant end-of-life care across settings, including home and facilities.

Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.

Responsibilities

• Complete visit types including hospice Face-to-Face visits within CMS timeframes.
• Collaborate with Hospice and Palliative teams to deliver patient-centered care.
• Prescribe medications and treatments within state scope and collaborative agreements.
• Order and interpret diagnostic tests and coordinate across care settings.
