Job summary

Gentiva seeks a Nurse Practitioner to provide dual hospice and palliative care, performing F2F visits for eligibility and recertification. The role blends autonomous practice with collaboration across IDG/IDT teams to support patients with serious illness in multiple settings.

Candidates should have an MSN and NP certification, plus emergency and regulatory knowledge; a PRN, flexible schedule may include days, evenings, and weekends. Comprehensive benefits accompany compensation.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited NP program.
• Current RN license and NP certification as required by state.
• Two years NP experience preferred; hospice/palliative care preferred.

Responsibilities

• Perform F2F hospice eligibility visits and recertifications per CMS timeframes.
• Collaborate with IDG/IDT to deliver patient-centered hospice and palliative care.
• Coordinate with PCPs and specialists to plan care and goals.
• Document findings accurately in EMR and ensure compliance.
• Participate in on-call coverage as needed.
