UnitedHealth Group's Amedisys hospice unit is seeking a dedicated Hospice RN On-Call to deliver after-hours clinical support. This role combines remote triage with occasional field visits to assess patients and coordinate care.

You will work with interdisciplinary teams to address urgent patient needs, educate patients and caregivers, and ensure accurate documentation. The position offers a comprehensive benefits package and opportunities for professional growth.

Qualificações

• Current RN license valid in the assigned state.
• Minimum 1 year of nursing experience, hospice/home health or acute care preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and vehicle insurance.

Responsabilidades

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
