Job summary

Peoples Health Services is seeking a professional nurse to join the interdisciplinary team to provide skilled nursing assessments and hospice care in patients' homes. The role includes evening, weekend, and on-call coverage to support the care plan and patient family needs.

Responsibilities include coordinating with physicians and community resources, maintaining accurate patient records, and delivering compassionate care aligned with agency policies and procedures.

Qualifications

• Possession of current license to practice as a registered professional nurse in Florida.
• Minimum two years of recent varied work experience as a professional nurse, preferably in acute, oncology, home health, or hospice settings.
• Ability to assess and respond to patient and family needs in varied settings.
• Skill in nursing practice and coping with family emotional stress.
• Availability to work evenings, nights, weekends, and holidays.

Responsibilities

• Provide skilled nursing assessments and hospice care in patients' homes.
• Cover evening, weekend, holiday, and on-call shifts as scheduled.
• Plan, implement, and evaluate nursing care using the nursing process.
• Coordinate with attending physicians and community resources.
• Maintain up-to-date patient records reflecting care plans and changes.
