Job summary
3HC Home Health & Hospice Care Inc. is seeking a Home Health and Hospice Nurse, RN, to cover after-hours weekend on-call from Friday 5pm to Monday 8am, providing skilled nursing care to Home Health and Hospice patients.
The role emphasizes one-on-one care, patient education, and coordination with the care team. Responsibilities include admissions, care planning, medication administration, and ensuring quality patient outcomes within MatrixCare workflows while maintaining compliance with agency
Qualifications
- Active RN license to practice in North Carolina.
- Minimum 1 year of professional nursing experience in acute care.
- CPR/BCLS certification current.
- Ability to communicate clearly with patients, families, staff and physicians.
Responsibilities
- Admit patients to the Agency and perform initial assessments.
- Develop and implement patient care plans with the care team.
- Provide skilled nursing care per physician plan.
- Educate and counsel patients and families on care needs and disease processes.
- Coordinate with supervising physicians and nursing staff to adjust care as needed.
