Phoenix Home Care is seeking an On-Call Registered Nurse to support our Springfield hospice team. This role delivers timely, skilled end-of-life care in patients' homes, emphasizing compassionate, patient-centered service.

The RN will respond to overnight and weekend needs, perform routine and as-needed visits, and collaborate with the interdisciplinary hospice team to ensure comfort and symptom management.

Qualifications

• Experience delivering hospice care in patients' homes or similar settings.
• Ability to work independently with strong clinical judgment.
• Excellent communication and patient- and family-centered care.

Responsibilities

• Respond to hospice patients and families overnight and on weekends.
• Complete routine and as-needed home visits in Springfield, MO.
• Triage urgent situations and coordinate with hospice physicians for symptom management.
• Collaborate with interdisciplinary hospice team and family caregivers.
• Maintain accurate clinical documentation and regulatory compliance.
• Participate in ongoing training and quality initiatives.
