Job summary

Suncrest Hospice seeks a Registered Nurse to provide compassionate in-home and facility-based care to a defined patient group. You will participate in the interdisciplinary team, perform psychosocial assessments, and coordinate care with families and other professionals.

Responsibilities include weekend coverage from 8:00 to 20:00, documenting visits in the EMR system, and ensuring adherence to hospice regulations while maintaining high-quality patient experiences across service areas.

Qualifications

• Active RN license in Illinois in good standing.
• Ability to work within interdisciplinary team.
• Capable of independent patient visits and case management.
• Excellent time management and communication skills.
• Experience with hospice rules and navigating EMR systems.

Responsibilities

• Provide patient care visits to home and facility settings.
• Participate in interdisciplinary team rounds and care planning.
• Manage on-call duties weekends 8am-8pm.
• Document patient visits and maintain accurate EMR records.
