Job summary

The Care Team Hospice Care is seeking an On-Call Registered Nurse to provide skilled nursing support for hospice patients in homes and facilities. This role offers after-hours and weekend coverage with a mix of scheduled visits and on-call responsiveness, primarily remote when not in the field.

You will assess patient conditions, manage symptoms, coordinate care with the interdisciplinary team, and educate patients and families about medications and end-of-life care.

Qualifications

• Active RN license in the state of practice required.
• Minimum of one year of nursing experience.
• Experience with EMR; Homecare Homebase (HCHB) experience preferred.
• Strong clinical assessment, symptom management and critical thinking.
• Excellent communication and interpersonal skills.
• Ability to work independently and prioritize urgent patient needs.

Responsibilities

• Respond to after-hours and weekend patient, family, and caregiver needs with timely nursing assessments and interventions.
• Perform on-call visits to assess changes in patient condition, manage symptoms, and adjust care plans.
• Coordinate care with physicians, caregivers, pharmacies, and interdisciplinary team members.
• Educate patients, families, and caregivers on medications, symptom management, and end-of-life care.
• Facilitate admissions, deaths, and urgent clinical situations per hospice policies.
• Document assessments and care coordination in the EMR accurately and timely.
• Deliver compassionate, patient-centered care addressing physical, emotional, and psychosocial needs.
