Qualifications

• Clinical confidence to triage and respond to a wide range of scenarios.

Responsibilities

• Be fully responsible for on-call coverage during your 7 days on.
• Respond to urgent patient needs including pain and symptom management.
• Provide telephonic support to families and caregivers.
• Conduct in-home visits across the service area.
• Maintain accurate documentation in electronic health records.
