Job summary

Amedisys is seeking a PRN Nurse Practitioner for home health in Alabama. This role involves evaluating patients in their homes, coordinating care with an interdisciplinary team, and delivering high-quality palliative and preventive services to patients in a comfortable setting.

Responsibilities include performing face-to-face visits, assessing clinical changes, ordering appropriate diagnostics, and referring to specialists as needed.

Qualifications

• Must hold current and unencumbered Nurse Practitioner license in the assigned state.
• Must have authority to practice and prescribe as a nurse practitioner in the state of service.
• Must have Medicare billing privileges as a nurse practitioner.

Responsibilities

• Communicate significant findings and changes in condition to the clinical manager/attending physician and interdisciplinary team.
• Complete face-to-face encounters and assess disease-related changes, functional decline, and quality of life.
• Order/review labs, imaging, EKGs and other diagnostics per patient condition and document treatment plan with physician review.
• Make referrals to other disciplines as indicated by patient needs.
• Participate actively in interdisciplinary team conferences.
