Job summary

Amedisys is hiring a Nurse Practitioner in Indiana to deliver in-home care and support patients where they are most comfortable—at home. This PRN role offers flexible scheduling and the opportunity to work with an interdisciplinary team to improve quality of life.

Responsibilities include assessing patients, ordering tests, coordinating with physicians, and providing compassionate palliation and symptom management. Licensure and prescribing privileges in Indiana are required.

Qualifications

• Current and unencumbered Nurse Practitioner license in Indiana.
• Must have authority to practice and prescribe as a NP in Indiana.
• Must have Medicare billing privileges as a NP.
• Must meet state collaborative requirements and provide physician agreements.
• Must maintain CPR certification.

Responsibilities

• Communicate findings and changes in condition to the clinical team.
• Complete face-to-face encounters and assess disease changes, function decline, pain and symptom management, and quality of life.
• Order and review labs, imaging, and diagnostics for palliation and treatment and document the plan.
• Make referrals to other disciplines as indicated and participate in team conferences.
• Provide comprehensive in-home NP care to patients and families.
