Job summary

Amedisys, part of the Optum family, is hiring a Nurse Practitioner to deliver compassionate palliative care in home health and hospice settings. You will collaborate with physicians to plan, implement, and adjust patient care plans while ensuring accurate documentation and compliance with laws and protocols.

Responsibilities include coordinating care across healthcare teams, educating patients and families, and maintaining required records.

Qualifications

• Graduate-level nursing degree and NP licensure.
• Certification by AANP or ANCC required.
• Authorization to practice and prescribe, with DEA if needed.
• Medicare billing privileges for NP.
• State collaboration requirements and documentation.
• State-required malpractice insurance or eligibility.

Responsibilities

• Provide palliative care in home and clinical settings with physicians and medical directors.
• Develop individualized care plans based on patient needs.
• Review treatments and tests; update plans with physician input.
• Complete documentation promptly and accurately.
• Coordinate care across providers and facilities and support education.

Education

Master's degree or higher in Nursing

Licensed as a Nurse Practitioner in the state where services are provided

Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners or ANCC

Authorized to practice and prescribe as a Nurse Practitioner in the state where services are provided

Medicare billing privileges as a Nurse Practitioner

Meets state-specific physician-Nurse Practitioner collaboration requirements

Maintains state-required insurance for Nurse Practitioner practice

Current CPR certification
