Job summary

Amedisys, part of the Optum family, is seeking a Per Diem Hospice Nurse Practitioner to provide compassionate, patient-centered care to individuals with life-limiting illnesses, focusing on comfort and quality of life. You will work with an interdisciplinary hospice team, handling assessments, symptom management, and family support in a home-based setting.

The role is PRN with flexible days; you will participate in face-to-face encounters, coordinate care, and contribute to end-of-life care

Qualifications

• Current and unencumbered Nurse Practitioner license in the assigned state.
• Current CPR certification.
• Authorized to practice and prescribe as a nurse practitioner in the state where services are provided.
• Medicare billing privileges as a nurse practitioner.
• Insurance coverage required by state law for nurse practitioners to practice or eligible via group coverage.
• Must meet physician-nurse practitioner collaborative requirements and provide active agreements; transferable terms.
• 1+ year experience as a Nurse Practitioner or 1+ year as hospice RN in last three years.
• Knowledge of Medicare reimbursement and coding for all levels of service.
• Driver's license and reliable transportation.

Responsibilities

• Communicate significant findings and changes to the clinical manager and interdisciplinary team.
• Complete face-to-face encounters and assess disease-related changes, decline in function, adherence and effectiveness of pain and symptom regimen, the patient and family coping mechanisms and quality of life.
• Order and review labs, X-rays, EKGs, and other diagnostic procedures for palliation and treatment; document the treatment plan and review results with collaborating physician.
• Make referrals to other disciplines as indicated by the patient's needs.
• Actively participate in interdisciplinary team conferences.
