Job summary

Del Corazon Hospice invites a qualified Nurse Practitioner to join our compassionate team in Albuquerque, NM. You will perform face-to-face visits, coordinate with our Medical Director, and ensure high-quality end-of-life care for patients and families.

Successful candidates hold an NM NP license, a Master's or Doctorate in Nursing, and at least one year of hospice or palliative care experience, plus current APRN certification.

Qualifications

• Master's or Doctorate in Nursing with current NP licensure in NM.
• Minimum 1 year of experience as an NP in a hospice or palliative care setting.
• Current Advanced Practice Registered Nurse (APRN) certification

Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources
