Del Corazon is seeking a compassionate Nurse Practitioner to provide hospice care in Santa Fe, NM. You will conduct face-to-face visits with patients in homes or facilities and collaborate closely with the hospice Medical Director to ensure comprehensive patient care.

The role emphasizes symptom management, family education, and empathetic support, with a flexible PRN schedule and compensation up to $200 per visit.

Qualifications

• Education: Master’s degree or Doctorate in Nursing with current NP licensure in New Mexico.
• Experience: Minimum of 1 year as an NP in hospice or palliative care.
• Certification: Current APRN certification.
• Skills: Strong clinical assessment and communication with empathetic patient care.
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement).

Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities.

We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Qualifications

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
