Job summary

Del Corazon is seeking a Hospice Nurse Practitioner (NP) in Albuquerque, NM to conduct face-to-face visits and work closely with the Medical Director to ensure comprehensive patient care.

The role emphasizes compassionate end-of-life care, clinical assessment, and symptom management in home or facility settings. A Master’s/Doctorate in Nursing with NM NP licensure, APRN certification, and hospice experience are required.

Qualifications

• Master’s or Doctorate in Nursing with current NP licensure in NM.
• Minimum 1 year NP hospice/palliative care experience.
• Current Advanced Practice Registered Nurse (APRN) certification.
• Strong clinical assessment and communication skills with empathetic patient care.
• Reliable transportation and valid auto liability insurance (mileage reimbursement provided).

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities to assess and support clinical needs.
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care.
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families.
• Document patient encounters accurately and timely to ensure regulatory compliance.
• Serve as a resource for clinical questions and assist in educating families and caregivers about end-of-life process and available resources.
