Job summary

Del Corazon is seeking a Nurse Practitioner to join its compassionate hospice team in Los Alamos, NM. You will conduct face-to-face visits with patients in homes or facilities, working closely with the Medical Director to ensure comprehensive care.

The ideal candidate has 1+ year hospice/palliative NP experience, an NP license in NM, and APRN certification. A flexible PRN schedule and competitive per-visit compensation are offered to support families during end-of-life care.

Qualifications

• Education: Master’s degree or Doctorate in Nursing with NP licensure in New Mexico.
• Experience: Minimum of 1 year of NP experience in hospice/palliative care.
• Certification: Current APRN certification.
• Skills: Strong clinical assessment and communication with empathetic patient care.
• Transportation: Reliable transportation and auto liability insurance (mileage reimbursement).

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities.
• Collaborate with hospice Medical Director and other healthcare professionals for optimal care.
• Evaluate/manage patient symptoms, adjust care plans, and provide emotional support to patients and families.
• Document patient encounters accurately and timely for regulatory compliance.
• Educate families and caregivers about end-of-life resources.
