Job summary

Del Corazon is seeking a compassionate Nurse Practitioner to provide hospice care in Santa Fe, NM. You will conduct face-to-face visits with patients in homes or facilities and collaborate closely with the hospice Medical Director to ensure comprehensive patient care.

The role emphasizes symptom management, family education, and empathetic support, with a flexible PRN schedule and compensation up to $200 per visit.

Qualifications

• Education: Master’s degree or Doctorate in Nursing with current NP licensure in New Mexico.
• Experience: Minimum of 1 year as an NP in hospice or palliative care.
• Certification: Current APRN certification.
• Skills: Strong clinical assessment and communication with empathetic patient care.
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement).

Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities.
• Collaborate with the Medical Director and other healthcare professionals for optimal patient care.
• Evaluate and manage symptoms, adjust care plans, and provide emotional support to patients and families.
• Document patient encounters accurately and timely for regulatory compliance.
• Educate families and caregivers about end-of-life resources and processes.
