Job summary

Del Corazon Hospice in Los Alamos, NM seeks a compassionate Nurse Practitioner to perform face-to-face hospice visits, collaborate with the Medical Director, and ensure comprehensive patient care for end-of-life needs.

Qualifications

• Master’s or Doctorate in Nursing with NM NP licensure.
• At least 1 year of NP experience in hospice or palliative care.
• Current APRN certification and strong clinical & communication skills.

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities.
• Collaborate with Medical Director and care team to ensure continuity.
• Evaluate symptoms, adjust care plans, provide family support.
• Document encounters accurately to meet regulatory requirements.
• Educate families and caregivers about end-of-life resources.
