Job summary

Del Corazon Hospice in Santa Fe, NM seeks a compassionate Nurse Practitioner to perform face-to-face visits, assess symptoms, adjust care plans, and work closely with our Medical Director to ensure high-quality end-of-life care. This PRN role offers flexible scheduling, up to $200 per visit, mileage reimbursement, and a supportive team dedicated to patient and family education.

Requirements include a Master’s or Doctorate in Nursing, NM NP licensure, APRN certification, and at least 1 year

Qualifications

• Master’s or Doctorate in Nursing
• Current NP licensure in New Mexico
• Strong clinical assessment and communication skills

Responsibilities

• Conduct face-to-face visits with hospice patients in homes or facilities
• Collaborate with Medical Director and healthcare team to ensure continuity of care
• Evaluate and manage symptoms and adjust care plans
• Document encounters accurately for regulatory compliance
