Job summary

Del Corazon is seeking a Nurse Practitioner to join its compassionate hospice team in Los Alamos, NM. You will conduct face-to-face visits with patients in homes or facilities, working closely with the Medical Director to ensure comprehensive care.

The ideal candidate has 1+ year hospice/palliative NP experience, an NP license in NM, and APRN certification. A flexible PRN schedule and competitive per-visit compensation are offered to support families during end-of-life care.

Qualifications

• Education: Master’s degree or Doctorate in Nursing with NP licensure in New Mexico.
• Experience: Minimum of 1 year of NP experience in hospice/palliative care.
• Certification: Current APRN certification.
• Skills: Strong clinical assessment and communication with empathetic patient care.
• Transportation: Reliable transportation and auto liability insurance (mileage reimbursement).

Responsibilities

We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources

Qualifications

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in New Mexico
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
