Advocate Health seeks a compassionate APRN to support the Hospice Care team with PRN visits, primarily in patients' homes or care facilities. This role emphasizes accurate documentation, care planning, and collaboration with nurses, physicians, social workers, and chaplains.

Schedule: PRN with a minimum of 2 shifts per month; no holidays and no routine weekend requirements. Pay range listed: $54.90–$82.35 per hour.

Competenze

• Active RN and APRN license in state(s) of practice with board certification.
• Master's Degree in Nursing required.
• NP/APRNP practice with hospice experience preferred.

Mansioni

• Conduct face-to-face hospice recertification visits in patients' homes and facilities.
• Perform head-to-toe assessments and evaluate disease progression.
• Review records and hospice plans of care for accuracy and compliance.
• Document thoroughly to support eligibility and recertification.
• Collaborate with interdisciplinary hospice team and participate in care planning.
