Job summary

UnitedHealth Group is seeking a Hospice Nurse Practitioner to provide palliative and supportive care consultation. This PRN/as-needed position involves face-to-face visits across a 50-mile service area with weekday hours and optional evenings/weekends.

Travel is required within the agency’s territory, with collaboration from the Hospice Medical Director and interdisciplinary team. Responsibilities include planning and coordinating care, documenting patient status, and guiding families.

Qualifications

• Active unrestricted license in the state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner.
• Current certification by a national body such as ANCC, NBCHPN, or AANP.
• 5+ years nursing experience.

Responsibilities

• Make face-to-face visits within prescribed time limits to hospice patients and report findings to Hospice Medical Director.
• Consult with Hospice Medical Director and collaborating physicians as needed.
• Initiate and coordinate the plan of care; document goals, interventions, and patient/family response.

Skills

Unrestricted state license

Nurse Practitioner or CNS

National certification (ANCC/NBCHPN/AN

5+ years nursing experience

CPR certification (preferred)
