Formación

• Active RN and NP licensure in state of practice.
• Board-certified in adult, family practice, or geriatric care.
• Ability to conduct comprehensive assessments and manage care plans.

Responsabilidades

• Conduct comprehensive adult/geriatric assessments.
• Evaluate and manage plan of care for hospice patients.
• Complete documentation in a timely manner.
