Douglas County Visiting Nurses is seeking a Hospice Nurse Practitioner to provide face-to-face assessments and coordinate care with the interdisciplinary team in Lawrence, KS. The NP may travel to patients' residences to deliver skilled nursing care, while maintaining EMR accuracy and complying with agency policies.
Compensation is based on experience and aligned with federal/state regulations; strong emphasis on quality assurance and team collaboration within the Hospice program.
The Hospice Nurse Practitioner provides face-to-face assessments and works with the interdisciplinary team (IDT). The Hospice NP is a conduit for a patient's ever-changing medical information, helping to ensure that each person is able to get the care they need. The Hospice NP may be responsible for traveling to a patient/client's place of residence to provide skilled nursing care. The Hospice NP is responsible for maintenance and accuracy of electronic medical records. The Hospice NP will provide services within the scope of Federal and State regulations, licensure, policies and procedures of the agency.

Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
