Job summary

Douglas County Visiting Nurses is seeking a Hospice Nurse Practitioner to provide face-to-face assessments and coordinate care with the interdisciplinary team in Lawrence, KS. The NP may travel to patients' residences to deliver skilled nursing care, while maintaining EMR accuracy and complying with agency policies.

Compensation is based on experience and aligned with federal/state regulations; strong emphasis on quality assurance and team collaboration within the Hospice program.

Qualifications

  • MSN or higher with one year's experience.
  • In good standing with licensing/certification agency and able to meet federal and state licensure requirements.
  • Current CPR certification or ability to obtain within 90 days of hire.

Responsibilities

  • Consult with attending physicians and Hospice Medical Director.
  • Collaborate with interdisciplinary team members on on-call basis for plan of care; help resolve conflicts in patient care decisions.
  • Review all admissions to hospice home care or inpatient settings in coordination with Hospice Medical Director.
  • Assist in developing and revising the plan of care for each patient/family/caregiver.
  • Make home visits as necessary.
  • Provide face-to-face assessments to determine service needs at set intervals as directed by the Medical Director.
  • Assist with review and revision of Hospice policies as needed.
  • Participate in establishing and providing continued support for all quality assurance standards and activities.

Job description

The Hospice Nurse Practitioner provides face-to-face assessments and works with the interdisciplinary team (IDT). The Hospice NP is a conduit for a patient's ever-changing medical information, helping to ensure that each person is able to get the care they need. The Hospice NP may be responsible for traveling to a patient/client's place of residence to provide skilled nursing care. The Hospice NP is responsible for maintenance and accuracy of electronic medical records. The Hospice NP will provide services within the scope of Federal and State regulations, licensure, policies and procedures of the agency.

NOTE: Actual compensation is based on experience.

DUTIES AND RESPONSIBILITIES:
  • Consult with attending physicians and Hospice Medical Director.
  • Consult with interdisciplinary team members on an on-call basis relating to the patient/client/family/caregiver plan of care; help resolve conflicting patient/client care decisions.
  • Review all admissions to hospice home care or in-patient setting in coordination with Hospice Medical Director.
  • Assist in developing and revising the plan of care for each patient/client and/or family/caregiver.
  • Make home visits as necessary.
  • Provide face-to-face assessments to determine need for services at specific intervals as determined by the Hospice Medical Director.
  • Assist with the review and/or revision of policies for Hospice as needed.
  • Participate in establishing and providing continued support for all quality assurance standards and activities.
QUALIFICATIONS:
  • Masters of Science in Nursing (MSN) or higher with one year's experience.
  • In good standing with discipline specific licensing/certification agency and able to meet all federal and state licensure requirements.
  • Current CPR certification or ability to obtain within 90 days of hire.
EXPERIENCE:
  • Basic level knowledge of EMR (electronical medical records) and email functions.
  • Certified in palliative care.
  • High level of medical knowledge and communication skills.

Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
