Job summary

Cherish Hospice Corp in Warsaw, IN is seeking a PRN Nurse Practitioner to provide essential clinical expertise and compassionate care to hospice patients on an as-needed basis. This flexible role supports the care team and helps ensure comfort during life's most sacred moments.

You will assess, treat, and document patient care while collaborating with RNs, social workers, and chaplains to maintain continuity of quality care across home or facility settings.

Qualifications

• Active Nurse Practitioner license is required.
• Valid driver's license is required.
• Current professional liability insurance is required.
• Minimum 1 year of clinical experience is required.

Responsibilities

• Perform comprehensive assessments and manage symptoms to ensure patient comfort.
• Prescribe medications and adjust care plans to optimize quality of life.
• Serve as a clinical resource to the interdisciplinary hospice team.
• Develop and adjust care plans emphasizing pain management and dignity.
• Collaborate with RNs, LPNs, social workers and chaplains to deliver holistic care.
• Document clinical encounters accurately and in compliance with hospice regulations.
