Job summary

Constellation Health is seeking a dynamic Hospice Nurse Practitioner to join our team in Cleveland, OH. The NP provides comprehensive hospice services under physician supervision, including assessments, care planning, and coordination with families and providers.

The ideal candidate holds an accredited NP program credential, a current DEA license, and state nursing licensure with CPR certification, plus hospice experience.

Qualifications

• Graduation from an accredited School of Nursing and accredited NP program.
• Current DEA license and state nursing licensure with CPR certification.
• Previous home care or hospice experience preferred.

Responsibilities

• Provide medical care for terminal illness under physician supervision.
• Perform history and physical exams and document findings in medical records.
• Develop individualized plans of care and prescribe medications as appropriate.
• Complete hospice face-to-face visits to assess eligibility for benefits.
• Coordinate with hospice interdisciplinary team to ensure seamless care.
