Ohio Living is seeking a Hospice Nurse Practitioner for Greater Cleveland. You will provide preventive medical, palliative, and educational services to patients in home health and hospice settings.

Responsibilities include comprehensive histories/exams, managing acute and chronic conditions, prescribing meds, ordering tests, counseling families, and ensuring accurate documentation. Ohio Living emphasizes compassionate care and teamwork.

Qualificações

• Current unencumbered Ohio NP license.
• Certificate of Prescriptive Authority or Externship Certificate to Prescribe.
• Two years experience in the home health and/or hospice field.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
• EMR experience preferred.
• Must be able to read, write, speak, and understand English.
• Must possess a valid driver's license and acceptable driving record.

Responsabilidades

• Conducts comprehensive medical and social histories and performs physical examination of patients in home health/hospice settings.
• Treats acute illnesses, chronic conditions and provides palliative care as appropriate.
• Orders and interprets lab tests and imaging.
• Provides information, instruction, and counseling on health maintenance and illness management.
• Refer patients to other health professionals as needed.
• Maintains accurate documentation and participates in quality improvement projects.
