Ohio Living Home Health & Hospice—Greater Youngstown is seeking a Hospice Nurse Practitioner to deliver patient care across home, facility, and clinic settings. The role emphasizes thorough assessments, diagnostic reasoning, and collaborative care planning in alignment with hospice standards.

Qualified candidates hold an Ohio NP license, NP certification, and prefer experience in geriatrics and home health. A valid driver's license and insurability are required; travel is involved as needed.

Qualifications

• Current unencumbered license to practice as an NP in Ohio required.
• Two years of advanced practitioner experience preferred; geriatrics experience preferred.
• Must read/write English; valid driver’s license and insurability.
• Home health/hospice experience is a plus.

Responsabilités

• Care and relationship management during home, facility, or clinic visits.
• Prescribes medications and interprets tests within scope of practice.
• Collaborates with physicians and other clinicians to form care plans.
• Maintains EMR documentation in a timely and accurate manner.
• Ensures compliance with federal, state, and local regulations.
• Supports program development and education initiatives.
